Below, you will find detailed information about our visit types, diagnostic services, and important appointment guidelines to help you prepare for your visit. Diagnostic Services

Appointments and Preparation

New vs. Established Patients

  • New Patient: You are considered new if you have not received professional services from Sugarloaf Medical P.C. within the last three years.
  • Established Patient: You are considered established if you have received care within the past three years.

Appointment Confirmation and Online Check-In

  • Promptly confirm your appointment upon receiving our SMS or email reminder.
  • Update your demographic, insurance, and preferred pharmacy information to expedite check-in.

Completing Required Forms

  • New Patients: Complete all new patient forms online at least 5 days before your appointment. Ensure previous medical records are transferred to our office.
  • Established Patients: Provide us with any external lab results or diagnostic test reports since your last visit. Update specialist information and bring any new or relevant documentation.

Fasting Instructions

  • Annual Visits & Specific Chronic Disease Management Visits: Fast (no food) for 8–12 hours beforehand; water and regular medications are allowed.
  • Non-Fasting Visits: No dietary restrictions required.
  • H. Pylori Testing: Fast for at least 2 hours prior, with no fluids allowed.

Examination Guidelines

  • Wear comfortable attire for quick and easy access to areas that may need examination.
  • Refrain from applying lotions or oils to areas scheduled for diagnostic evaluation.
  • For Pap smears, please reschedule if your appointment falls during your menstrual cycle.

Follow Up Care

For optimal and personalized care, please ensure that all follow-up and preventive care visits are scheduled at check-out after each office appointment.

If you have additional concerns, questions, or require further clarification after your appointment—or if you did not schedule your next visit during check-out—it is important to promptly arrange an office visit. To maintain patient safety and provide comprehensive care, any issue requiring provider assessment, medical decision-making, or detailed counseling should be addressed during a scheduled visit. If you call our office with clinical concerns, our staff will assist you in scheduling an appointment directly with your provider.

Please do not direct clinical inquiries to medical assistants, administrative staff, or registered nurses, as all medical decisions and assessments are exclusively handled by our physicians and advanced practice providers.

Appointments Are Required For (But Not Limited To)

Management and Monitoring of Chronic Medical Conditions

  • Routine Management of Stable Chronic Diseases: Routine follow-up visits every 3–6 months, based on your personalized care plan, to evaluate your condition, review medication effectiveness, and make necessary adjustments.
  • Management of Unstable Chronic Diseases: Schedule a visit if your chronic condition changes, worsens, or if you have concerns requiring closer monitoring, medication adjustments, or changes to your treatment plan.

Discussion and Adjustment of Treatment Plan and/or Follow-Up Testing Based on Lab and Diagnostic Results

  • Review and Discussion of Lab Results: An office visit is necessary for a thorough review of your lab results—abnormal or otherwise—along with counseling, possible treatment plan adjustments, or additional follow-up testing as needed.
  • Review and Discussion of Diagnostic Test Results: An office visit is also required to interpret results from X-rays, MRIs, CT scans, ultrasounds, or other studies, discuss any changes to your care plan, and arrange further testing if indicated.

Medication Management and Counseling

  • Prescription Refills and Medication Adjustments: An office visit is necessary when you need prescription refills, dosage changes, or new medications, as refills or changes may not be granted without proper evaluation to ensure patient safety.
  • Evaluation of Non-Emergent Medication Side Effects or Effectiveness: Schedule an appointment if you experience non-emergent side effects, persisting symptoms, or concerns about your current medications. If you have a life-threatening emergency, please call 911 or go to the nearest emergency room.
  • Changes in Insurance or Authorization Affecting Medication or Treatment: Schedule an appointment to discuss changes in your insurance coverage, medication affordability, or prior authorization requirements so we can adjust your treatment plan accordingly.

Coordination of Specialist Referrals

  • Specialist Referrals and Care Coordination: A referral requires an office visit to discuss the reason for the referral, recommend the appropriate physician or practice, coordinate care, share relevant results, and arrange any needed physician-to-physician communication. Some insurance plans also require prior authorization and medical notes to approve the referral, necessitating an office visit.

Forms Requiring Physician Assessment or Examination

  • Completion of Health-Related Forms: An office visit is required for physician-completed forms such as assisted living forms, health and wellness forms, or employer-requested documentation. We must evaluate your current health status to accurately complete these documents.
  • FMLA, Disability, and Other Specialized Forms: Our practice does not typically complete FMLA, disability, or similar forms for conditions not directly managed by our physicians. These forms should be completed by the specialist overseeing the specific condition. If you require disability-related documentation for a condition beyond our scope, we will refer you to the appropriate specialist who can address your needs.
