Patient Portal is a secure, convenient, and easy way to access your health information

• Appointments:
  Easily schedule new appointments or reschedule existing ones to fit your needs.

• Medical Records:
  View detailed summaries of your visits, access lab results, and review treatment plans at your convenience.

• Medications:
  Keep your medication list current by reconciling any changes. Request prescription refills securely and directly through the portal.

• Communication:
  Send and receive secure messages with your healthcare team. Access personalized health reminders and educational materials to help you manage your wellness.

• Personal Information:
  Update your demographic, insurance, and contact details. You can also review and pay your medical bills online.

• Laboratory and Test Results:
  Access your laboratory and diagnostic test results as soon as they are available.

• Download Healow App:
  Enhance your portal experience by downloading the Healow app from the Apple App Store or Google Play. Use the Practice Code JAFEAA to log in.

Account Setup

• After registration, you will receive an email invitation to set up your Patient Portal account. Follow the provided link to create your username and establish a secure password.

Logging In

• Log in using User Credentials or opt for the Use Mobile Phone login method.

Username & Password Reset

• If you encounter login issues, click "Trouble logging in?" on the login page.

  • Forgot Username:

    • Enter your First Name, Last Name, and Date of Birth, then submit the form. An email will be sent to your registered email address.

  • Forgot Password:

    • Enter your Username to receive email instructions for securely resetting your password.

• If your account locks after three unsuccessful login attempts, please contact our office.

• Always log out at the end of each session.
• Never share your login credentials.
• Regularly update your password.
• Notify Sugarloaf Medical P.C. immediately if you suspect unauthorized access.
• Avoid using public Wi-Fi when accessing sensitive health information.
• Secure your personal devices and refrain from storing medical communications on employer-provided devices.
• Use screensavers or close messages to prevent unauthorized viewing

Sugarloaf Medical P.C. strictly adheres to the Health Insurance Portability and Accountability Act (HIPAA). Your information is encrypted, secure, and accessible only to authorized personnel.

• The Patient Portal is intended for non-emergency communication only.

• In an emergency, call 911 or visit the nearest emergency room immediately.

• Portal communications are monitored during regular business hours only (excluding after-hours, weekends, and holidays).

• Please allow three business days for a response; if you do not receive one, contact our office directly.

• Follow-up on appointments, recommendations, abnormal results, and sensitive issues remains your responsibility.

• Information provided through the Portal is intended to support, not replace, direct medical consultation.

• Sugarloaf Medical P.C. is not liable for any damages arising from portal use, including system outages or security breaches.
